SHORT TERM CARE

Rehabilitation Services

At Yorkview, our team believes that rehabilitation means restoring lost abilities and regaining maximum self-sufficiency after an illness or injury. This includes treating the whole patient; physically, emotionally, and socially, and returning them to their optimum level of function for a better quality of life.

The residents at Yorkview are assisted in their recovery in a home-like environment by a team of caring and highly qualified rehabilitation professionals.

We offer a wide range of specialized programs, including but not limited to:

  • Traditional rehab services for strengthening gait training, Activities of Daily Living (ADL’s), speech, and swallowing interventions, body mechanics and functional limitations in self care
  • Pain management and fall prevention programs
  • Ultrasound, E-Stim, Diathermy, and additional physical agent modalities
  • Car transfers and home safety assessments
  • Cognitive skills development
  • Receptive/expressive language training
  • VitalStim Technology
  • Integration of respiratory training techniques to compliment Activities of Daily Living, mobility, and positive outcomes

 

Total Joint Replacement

This program focuses on using physical and occupational therapies to maximize the function of residents recovering from total knee and hip replacement surgery. Our goal is to give residents the highest level of care so that they can be prepared to return home and get back to the activities they love to do.

Communication and Swallowing

Our program offers specialized speech pathology services that enhance a resident’s ability to communicate or swallow safely following a stroke or when affected by brain damage, dementia, oral cancer, parkinson’s or other diseases.

DISCHARGE PLANNING

Our interdisciplinary team provides supportive discharge planning from the day of admission through the day of discharge and beyond. This effort, led by the social service worker, is focused on coordinating all care and services with your health care. We will work directly with your home care team to ensure that your post discharge care needs are met timely.

Pulmonary Focused Rehabilitation

Our pulmonary focused rehabilitation program is staffed with a licensed respiratory therapist to treat complex respiratory problems including patients with new tracheostomy.

Our consultant pulmonologist provides clinical oversight of our pulmonary focused program in collaboration with the respiratory therapist and nursing staff. Our goal is to provide the highest-level quality care to high acuity respiratory patients. Early identification of symptoms paired with quick intervention supports our goal of reducing unnecessary hospitalizations for this high-risk population.

Pulmonary Rehabilitation Program

Yorkview offers an interdisciplinary, collaborative approach to specialized pulmonary care by providing a respiratory therapist 7 days a week with a consulting pulmonologist to address the needs of the high acuity patients and reduce re-hospitalizations.

Our respiratory program provides care to patients with complex pulmonary care requirements, tracheostomy and other pulmonary needs due to various conditions including:

  • Respiratory Failure
  • COPD
  • Lung Disease or Chest Injury
  • Congestive Heart Failure
  • Pneumonia
  • Asthma
  • Pulmonary Hypertension

LONG TERM CARE AND MEMORY CARE

For those residents who are unable to return home, Yorkview provides an on-going plan of care based on the individualized care needs of each resident. Services may include specialized dementia programming, nutrition management, wound care, assistance with activities of daily living, and other supportive care depending on the individual needs of the resident. We offer support and guidance to families who are encouraged to be involved in the care needs of their loved ones.

Secured Dementia Care

Caring for a spouse, parent, or loved one with alzheimer’s, dementia, or memory loss is rewarding but comes with challenges and requires the caregiver to have patience, flexibility, and understanding on a daily basis. Many times this leads to the caregiver seeking out assistance and support to ensure the loved one’s dignity, quality of life, and well-being are maintained.

Yorkview Nursing and Rehab offers quality of life and well-being for those residents who do suffer from memory loss in a safe and secured environment. We offer a secured unit where our residents with memory loss such as dementia and alzheimer’s, can safely walk around whenever they want 24/7. Our family members can be rest assured that their loved ones will be able to move about as they wish, even if they were to get up in the middle of the night. Our trained and dedicated staff will be there to offer hands on support and provide care for their ongoing needs, whenever that might be, anytime of day.

Each resident will have an individualized care plan developed based on their specific care needs. Our interdisciplinary team will work with each resident and their loved ones to ensure they are safe, maintain their dignity, and are actively engaged in their activities of daily living. This allows our residents to stay as independent as possible while having the added support when it’s needed. We also continue to work with our families to get insight on their loved one’s past history such as their career, hobbies, food choices, and daily routine to further meet their individualized needs.

Our memory care unit offers activities that are geared toward stimulating the resident’s brain, physical movement, and social needs. We encourage all of our residents to participate in activities to enhance their quality of life. While we encourage activity participation, we understand the need or desire to be in a quiet space as well and will direct those residents needing this time to dedicated quiet spaces.

ACTIVITIES

Our activities calendar offers robust activities that provide our residents with an opportunity to socialize, engage their brains, and stay fit. There are inviting lounge areas for quiet reading, or dedicated activity spaces for those wanting to enjoy all of the action.
We offer fitness classes geared towards those residents needing to maintain their range of motion and stay active. There are other various activities such as art classes, bingo, musical guests, and more. We also schedule outings to shops and stores in the area for those wanting to pick up a few items. Our activities staff are there to ensure there is engagement at all levels for our residents.

Pet Therapy

Using trained service animals, our residents are offered emotional support through the use of interaction with a pet. The use of therapy animals offers help with coping and recovery from health problems, whether short term or long term. Our goal is to have our residents show improved mood when getting a visit from a friendly pet.
